f) Eine eidesstattliche Erklärung, dass die Informationen in Ihrem Schreiben den Tatsachen entsprechen und Sie der Urheberrechtsinhaber sind oder befugt, in dessen Namen zu handeln.

Bitte nehmen Sie desweiteren davon Kenntnis, dass ungerechtfertigte Abmahnungen und unhaltbare Forderungen, die auf falschen Behauptungen beruhen, einen Straftatbestand darstellen.
